Agreement to Comply with Safe Data Handling Procedures
I, the Coach-in-Training, agree to comply with the safe data handling procedures outlined by the General Data Protection Regulation Act (GDPR), the Health Insurance Portability and Accountability Act of 1996 (HIPAA), and the Practice Client Enrollment Form, which I agree to transmit to my Practice Client(s) for the purpose of obtaining the Practice Client’s consent to the collection of their personal data in connection to the Erotic Blueprint Coach Certification Program Training and Assessment process.
I agree to provide my Practice Client with the ‘Practice Client Enrollment Form’ so that my Practice Client fully understands what data will be collected, how it will be stored and securely disposed of, and their rights pertaining to such data. If my Practice Client consents to the terms of the Practice Client Enrollment Form, I agree to ensure that the form is signed and returned to Jaiya, Inc. (hereinafter ‘The Company’).This agreement may be assigned to one of The Company’s assigns or subsidiaries. The obligations of the Coach-In-Training as outlined in this agreement will pass, in full, to the new company or subsidiary, should there be a transfer of the Company’s tangible and/or intangible assets to new companies or subsidiaries.
I understand that The Company shall in no way be held responsible for any failure on my part to comply with these safe data handling procedures, nor shall The Company be held responsible if I fail to provide my Practice Client with the Practice Client Enrollment Form.
Any agreements made separately between the Coach-in-Training and the Practice Client(s) for additional services which require the safe handling of data do not affect the obligations made herein.
